Deadline for submission: Tuesday, February 1, 2011
Applications must be completed and submitted to the Canadian Diabetes Association via courier (overnight/next day only) by Tuesday, February 1, 2011.
Hand delivered applications must be received at the CDA National Office in Toronto, no later than 4:30 pm (EST) on February 1, 2011.
